Case Counselor Jobs
A case counselor provides individual and group counseling to clients and family members. Case counselors work with clients to develop individualized service and treatment plans with measurable goals, including substance abuse treatment, housing, relapse prevention, vocational referrals, and mental health assessments. Case counselors  assist with intake, assessment, and follow-up to ensure clients’ adherence to the treatment plan. Case counselors should maintain confidential client records and prepare client progress reports;  participate in case conferences and treatment team reviews of client progress; attend in-service trainings, community forums, and education sessions; and provide general administrative office support as needed (e.g., program reports). Case counselors provide client monitoring and case management and assist in community outreach and referrals and client follow-ups. Case counselors establish community relationships and connections with other service providers. They must embrace and embody the mission, vision, guiding principles, clinical vision, and goals of the company and perform any other duties as assigned.
Case counselor positions require a bachelor's degree in health and human services, social work, psychology, or other related fields, or equivalent experience. Case counselors should have two years of experience, education, or training in community health services and management with diverse client populations, specifically, but not limited to, the Asian and Pacific Islander community. Case counselors should have the ability to work flexible hours and travel as needed (especially some evening and weekend hours).
