\u003cp\u003e\u003cstrong\u003eAbout the Program: \u003c/strong\u003eThe San Diego BHC program is an active partnership between Telecare and the County of San Diego, superior court, the public defender and district attorney\u0026rsquo;s offices, and the probation department. Members are able to begin ACT services as soon as they are released from jail and are required to commit to the program for 18 months in order to participate. The primary focus of the program is to ensure members are set up with the proper resources and tools they will need to move forward without re-offending.\u003c/p\u003e

\u003cp\u003eUnder direct supervision, serves as a primary support and case manager for individuals with mental illness and/or co-occurring disorders by assisting them to live as independently as possible in their chosen community.\u003c/p\u003e

\u003cli\u003eConsistent with area of expertise, provides specialized services and interventions related to substance use, including thematic groups, evidence-based practices, and collaboration with external agencies and resources\u003c/li\u003e
\u003cli\u003eFacilitates groups according to the needs and interests of members served, especially in the area of education or skill-building related to Substance Use\u003c/li\u003e
\u003cli\u003eProvides appropriate interventions in crisis situations\u003c/li\u003e
\u003cli\u003eOn a rotating basis, performs officer of the day functions for both wellness, duty and stand by duty tasks\u003c/li\u003e

\u003cli\u003eMust possess at least 1 year of experience providing services as a Substance Use Specialist\u003c/li\u003e
