\u003cp\u003e\u003cstrong\u003eAbout the Program\u003c/strong\u003e: We believe recovery starts from within, and that our job is to do whatever it takes to provide the support needed on your recovery journey in a comfortable, structured setting. Our 12-bed crisis residential treatment (CRT) includes a full team of nurses, psychiatrists, clinicians, peer specialists, and recovery specialists providing support 24 hours a day.\u003c/p\u003e
\u003cp\u003eOur services emphasize choice-making skills and harm-reduction techniques. Staff work with you within your cultural dynamic in building independence and self-responsibility in order to nurture your recovery and successfully transition you home or to less restrictive community settings.\u003c/p\u003e

\u003cp\u003e\u003cstrong\u003eESSENTIAL FUNCTIONS\u003c/strong\u003e\u003cbr /\u003e\u0026bull; Demonstrates the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders\u003cbr /\u003e\u0026bull; Provides safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care\u003cbr /\u003e\u0026bull; Establishes, maintains, and co-facilitates a therapeutic relationship with members served\u003cbr /\u003e\u0026bull; Provides case management, schedules appointments, communicates with conservators and treatment teams, arranges and conducts group outings and activities\u003cbr /\u003e\u0026bull; Provides daily living assistance including promoting and assisting members served in developing self-help skills\u003cbr /\u003e\u0026bull; Orders, logs and distributes medication\u003cbr /\u003e\u0026bull; Maintains the inventory and orders equipment and supplies for activity programs, as directed\u003cbr /\u003e\u0026bull; Drives members served in the Program van on outings\u003cbr /\u003e\u0026bull; Completes incident reports and forwards to Supervisor in compliance with established policies and procedures\u003cbr /\u003e\u0026bull; Notifies appropriate agencies of any known or observed incidents of abuse\u003cbr /\u003e\u0026bull; Assists with admissions and discharges\u003cbr /\u003e\u0026bull; Provides medical assistance: takes temperatures, blood pressure, pulse and provides First Aid when needed\u003cbr /\u003e\u0026bull; Observes, records and reports members served behavior as per applicable State, County and Telecare guidelines\u003cbr /\u003e\u0026bull; Reviews and implements members served service plans\u003cbr /\u003e\u0026bull; Actively participates in team meetings and service planning meetings as directed\u003cbr /\u003e\u0026bull; Demonstrates knowledge of management of assaultive behavior techniques\u003cbr /\u003e\u0026bull; Responds to emergency/crisis situations\u003cbr /\u003e\u0026bull; If employed at Inpatient program, must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training\u003cbr /\u003e\u0026bull; If employed at Outpatient program, must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training\u003cbr /\u003eDuties and responsibilities may be added, deleted and/or changed at the discretion of management.\u003c/p\u003e
