\u003cp\u003eAs part of the Telecare Family, the Kaiser Intensive Community Treatment (ICT) program will be an Assertive Community Treatment (ACT) program serving up to 75 Clients with Serious Mental Illness (SMI) throughout the Fairfield/North Bay Area.\u0026nbsp;\u003c/p\u003e
\u003cp\u003eAt Kaiser ICT, we partner with Kaiser Permanente psychiatrists and clinical staff to ensure successful engagement in treatment services. Our multidisciplinary team will provide short‐term case management and services to help members navigate the mental health system.\u003c/p\u003e
\u003cp\u003eOur program uses a community-based model where we connect members to the proper resources in order to help them achieve their wellness goals. All services are recovery-focused, strengths-based, and anchored in recovery principles.Upon arrival, a diverse clinical team will provide an individualized assessment and work with members every step of the way to support their goals in life.\u003c/p\u003e
\u003cp\u003eThe program is designed to meet the needs of Kaiser members who are high users of inpatient psychiatric services or who are at high risk of acute psychiatric hospitalizations. Services are provided in the community.\u003c/p\u003e

\u003cp\u003ePOSITION SUMMARY\u003cbr /\u003eUnder direct supervision of the Clinical Director/Administrator, this entry-level licensed management position supervises multidisciplinary teams, coordinates service needs, and collaborates with other services and agencies. Completes assessments and determines the appropriate level of services for new members. \u003cbr /\u003e\u003cbr /\u003eQUALIFICATIONS\u003cbr /\u003eRequired: \u003cbr /\u003e\u0026bull; One (1) year of experience in mental health setting \u003cbr /\u003e\u0026bull; Current and active BBS number in the state of CA (e.g. ASW, AMFT, APPC, LCSW, MFT, Psy D, PhD, LPCC, LMSW, etc)\u0026nbsp;\u003cbr /\u003e\u0026bull; One (1) year supervisory experience or supervisory training within six (6) months of employment\u003cbr /\u003e\u0026bull; Sensitivity to multi-cultural populations and issues\u003cbr /\u003e\u0026bull; Must be at least 18 years of age\u003cbr /\u003e\u0026bull; Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment \u003cbr /\u003e\u0026bull; All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification.
