Family Support Partner - Youth Specific FCCP

POSITION SUMMARY:
P
lans and implements delivery services through the Family Care Community Partnership (FCCP). Families served by this program have children under the age of 18 (for youth transition from the Youth Development Program, age 21) who are at risk for developmental, behavioral, and social problems, as well as families who are at risk for abuse or neglect. The Family Support Partner acts as a peer mentor and support to families, as a member of the Wraparound team, helping them to navigate systems, access resources and connect to natural supports.
This position is Youth Specific Support. For youth referred for Wayward or Youth diversion, Weekly contact with the youth is required. This support partner will mentor youth, spending time with youth building rapport, building skills or completing tasks to address needs and goals identified by the FCCP plan. Youth support FSP would also coordinate with FSCC and other team members especially parents to increase cooperation and communication. FSP will be competent to cover typical FCCP cases as well.
The Family Support Partner works under and receives regular direction (weekly supervision) from a program Clinical Supervisor or designee, according to the Agency’s mission and philosophy, Program and Division goals.
EDUCATION AND TRAINING REQUIRED:
High School diploma or equivalent. Experience in the human services or related field preferred. Personal experience with child welfare and/or mental health systems is required.
Demonstrated ability to work independently and within a team framework. Openness to ongoing supervision, training, wraparound certification and professional development.
Excellent written and verbal communication and personal boundaries. Ability to engage young people and their families.
WORKING CONDITIONS:
· Provide services in clients’ homes, in the community, and within the agency. Home/community visits may be in at-risk areas and of various conditions.
· Must be able to walk, climbing stairs and sit for extended periods of time.
· Provide own reliable transportation and car insurance.
· Communicate with clients and colleagues in person and via phone.
· Provide role-modeling and education on parent-child relations and family life skills.
· Establish and maintain professional relationships with clients and community providers.
