These individuals may also be homeless or have been homeless for a total of 120 days during the last twelve (12) months.\u003c/p\u003e
\u003cp\u003eOlder adult residents of Santa Clara County, ages 60 and older, who have been diagnosed with a serious mental illness, have a co-occurring substance use condition, and require intensive outpatient services.\u003c/p\u003e

We have a full staff of peer specialists, a psychiatrist, a nurse, a master\u0026rsquo;s-prepared team lead, and personal service coordinators with experience in both drug and alcohol and mental health, who are all here to promote a program culture where resilience and hope can flourish, and losses can be recovered.\u003c/p\u003e

Assists Administrator with quality improvement duties.\u003c/p\u003e
