OBRA Social Worker
Pay Range: $53,618.73 - $65,991.62
GENERAL STATEMENT OF DUTIES:
Under general supervision of Mental Health Supervisor (Health Services), implements SCCMHA requirements of Omnibus Budget Reconciliation Act of 1987 (OBRA) and Pre-admission Screening Annual Resident Review (PASARR). Serves as a member of the evaluating team to implement the purpose of OBRA/PASARR process. Demonstrates knowledge of Michigan Department of Health and Human Services (MDHHS) and OBRA/PASARR regulations. Participates in collaboration as a team member of an interdisciplinary assessment for those we serve, concerning placement that best meets their medical, behavioral and psychiatric needs in the least restrictive living arrangement. Serves as a collaborative partner with Saginaw County hospitals, nursing facilities, homecare agencies and community medical providers for the purposes of the OBRA/PASARR process. Provides Specialized Services including Nursing Home Mental Health Monitoring, OBRA Outreach and Psychotherapy to individuals while residing at the Nursing Facility. This position will be knowledgeable about and will actively support culturally competent recovery-based practices; person centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process.
MINIMUM REQUIREMENTS:
Education:
A Master's Social Worker, Professional Counselor, or Psychologist from an accredited school with ability to obtain clinical licensure.
Valid Michigan Driver's license with a good driving record.
Must be able to apply and obtain a licensed master's social worker (LMSW), licensed professional counselor (LPC), or licensed psychologist (LP) within six (6) months upon offer of employment.
Experience:
A minimum of three (3) years progressively responsible professional mental health experience as a clinician, counselor or therapist. A minimum of one (1) year experience working as an interdisciplinary team member in a nursing facility or acute care hospital setting. Prefer experience working with aging adults, mental health populations, including but not limited to, developmental disabilities or severe mental illness. Preference will be considered for those with OBRA/PASARR experience.
Knowledge, Skills and Abilities:
Knowledge of mental health programs, aging adults, developmental disabilities, mental illness, behavioral disorders, delirium, and neurocognitive disorders.
Knowledge of medical conditions and terminology.
Knowledge about third party payer requirements, Medicaid Provider Manual and the Michigan Mental Health Code.
Excellent professional writing and grammar skills.
Ability to relate to all segments of the community.
Ability to work with and assess diverse populations within the community.
Ability to serve public in a pleasant, courteous manner, and to communicate with people effectively.
Respectful of all socioeconomic and cultural backgrounds.
Excellent receptive and expressive communication skills (verbal and written).
Excellent assessment and diagnostic skills in behavioral health areas.
Ability to use personal computer-based data entry and use computer office applications.
Ability to plan, organize, and complete work in a timely fashion.
Ability to function autonomously.
Be knowledgeable of various computer programs, including but not limited to Microsoft Word, Microsoft Outlook, Microsoft Access, Microsoft Excel.
Ability to maintain favorable public relations and collaborate with community partners.
Ability to exercise independent and mature judgment.
Ability to accept and implement constructive supervision.
Ability to maintain appropriate professional boundaries.
Ability to effectively handle conflict with community agencies, consumers, guardians, and peers.
