Position Summary: This position supports the mission of Wesley Medical Center in optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Social Worker Case Manager is responsible for assessing the psychological, social, cultural, and/or financial needs that impact the high risk patient's health and recovery. Has the ability to utilize social services, counseling and clinical knowledge in the assessment of these needs. Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources. In collaboration with the RN Case Manager, patients will be assessed to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed throughout the course of hospitalization. Knowledge of payer source implications on discharge plan, resources and placement options to maximize patient’s successful dismissal.','!*!Required Registration, Licensure or Certification  Current licensure as a social worker by the Kansas Behavioral Sciences Regulatory Board as a LBSW  Current Basic Life Support (BLS) verification course, as specified in policy Preferred Registration, Licensure or Certification  None Required Education or Experience  Bachelor's degree in social work from an accredited institution  3- 5 years of recent clinical (community or health care) social work experience Preferred Education or Experience  Recent hospital social work or case management experience Required Knowledge and Skills  Ability to apply critical thinking skills  Demonstrates self-direction and motivation  Demonstrates highly developed written and oral communication skills  Demonstrates assertive interpersonal skills  Practices positive group dynamics and relationship building skills  Demonstrates accomplished presentation skills  Working knowledge of performance improvement  Demonstrates basic computer skills  Demonstrates an ability to affect change Preferred Knowledge and Skills  None ','Case Manager Social Worker - Part Time
