SENIOR MENTAL HEALTH SPECIALIST

Job Description:
Conduct assessments of clients referred to HRA funded WeCARE Program near Fordham Road and Grand Concourse, Bronx, NY.
Key Responsibilities:
1. Interview clients, review records, and confer with other professionals to evaluate individuals' mental and physical condition and to determine their suitability for participation in a specific program track including Wellness, Federal Disability, employment with accommodations, or employment without limitations.
2. Conduct the Social Security Administration Sequential Evaluation as part of the assessment process.
3. Develop client treatment plans based on research, clinical experience, and client histories.
4. Review and evaluate clients' progress in relation to measurable goals described in Individual treatment plans during re-assessments.
5. Review and evaluate client Functional Capacity Outcome during re-assessment when new medical and/or behavioral health information is provided
6. Initiate referral to psychiatrist, psychologist, review physician, physician assistant, CASAC or other health professionals as indicated during the assessment in order to complete the assessment in compliance with HRA policies and procedures.
Skills:
1. Able to assess Medical and behavioral health diagnoses and impact on individual's functional capacity.
2. Knowledge of medications, side effects, and impact on individual's functional capacity.
3. Requirements to qualify for Federal Disability Benefits.
4. Able to administer the SSA Sequential Evaluation
5. Able to use multiple software systems to review and assess medical and behavioral health conditions as well as document assessment and outcomes in electronic medical record
EXCELLENT TIME MANAGEMENT AND WRITING SKILLS A MUST
SPANISH SPEAKING highly desirable

 

Master's Degree Required, Minimum 3 yrs exp.

NYS professional license  LCSW or LMHC, Certified Rehabilitation Counselor (CRC), or other credentialed professionals as may be approved by HRA.
